How to Add and Setup a Contact Form on Squarespace

• Create a new section

• Click “Add Block”

• Select “Form” on the right side of the column

• Click on the “Design” tab to change the design of the form

• Click on “Storage” to change where the form submission notifications get sent to

To edit the options on the form:

• Click on the form and click the pencil option to edit the form

• Click “Edit Form Fields”

• Then add or delete fields as needed

• Click on the “Design” tab to change the design of the form

• Click on “Storage” to change where the form submission notifications get sent to
